Trip Envelope - One Trip Envelope is required to contain all the information about the things listed above between the time the driver starts his tour of duty and the time it ends.  If a driver needs more space on an envelope, two envelopes may be used for the one trip.  Trip envelopes are to be turned in with all documents required at the  Fontana, CA, terminal, at the end of each tour of duty.


At the top of the Trip Envelope, enter the  Driver name, Tractor number, Date, Starting Location, Tractor beginning Odometer mileage and number of Empty Pallets, if any, that are in the trailer.  


In the center section of the Trip Envelope enter all Trip Numbers, Trailer numbers, locations and details of freight loading and unloading. 


At the bottom, enter the End of run Trailer number, the End of run Date, the End of run location, End of Run Odometer mileage and the number of empty pallets being returned if any.


Use the back of the Trip Envelope to show routes traveled, Locations/Cities and odometer mileage when crossing state lines.


The sequence of loading and unloading locations on the Trip Envelope should agree with the entries on the Drivers Daily Log.
