•Adults between 18 and 65 years of age
•NO health insurance, including Medicaid or Medicare, VA benefits
•Resident of Craven, Pamlico or Jones counties in North Carolina
•Household income no greater than 200% of the federal poverty level
•Proof of income to include most recent 1040, W-2 forms, pay stubs and other financial information as requested.
