When booking a service for the first time, you will be asked to fill out a contract and form telling us a little bit about yourself so that we may treat your needs properly and efficiently. The form and contract will be kept on file for future appointments although once a year we will ask for an updated form in case of new information and/or changes in your health. It is forbidden to solicit our practitioners for services not booked through our company. If such actions occur where the practitioner is solicited and booked without the use of Buddha-To-Go’s booking services, it will result in banning the customer’s further use of our services and loss of that practitioners position with us.
