What to bring to your appointment: A current, valid California ID, such as a driver’s license. If you don’t have a driver’s license, please bring another current photo ID AND proof of California residency (e.g., utility bill, bank statement, phone bill, pay stub, car registration, etc.) Medical records or other documentation supporting the diagnosis of your medical condition.*** Examples of acceptable documentation include, but are not limited to, the following: A note or a copy of medical records from your primary care provider (PCP) which confirms your diagnosis. The PCP may be a medical doctor, acupuncturist, homeopath, naturopath, herbalist, chiropractor, physical therapist, podiatrist, licensed massage therapist, and other allied health care professionals. Radiology, such as X-rays or MRIs preferably with the accompanying written report OR if the report is not available, the diagnosis must be obvious to a non-radiologist, such as metal hardware placed to support a bone during an orthopedic surgery. Prescriptions that were written for the medical condition for which you would like to use medical marijuana. Recent prescription bottles with all identifying information intact that are for the medical condition for which you would like to use medical marijuana. Legal documents which clearly state the condition or incident for which you would like to use medical marijuana.
